Employees with permanent physical disabilities must request consideration for handicapped parking annually by completing an application form and forwarding copies to Employee Occupational Health and Wellness (EOHW) and the Disability Management System. After review of the form, EOHW advises Parking and Transportation Services of their recommendations. PTS then contacts the employees with information on how to obtain their permits.
